2013-2014 Accomplishments

  • Foundation was established and Board of Directors begun

  • Facebook audience grew to over 14,000 fans

  • Twitter audience grew to over 140,000 followers

  • Worked in conjunction with American Heart Association to pass the CPR in Schools Legislation, making Oklahoma the 16th state with a mandatory training law

  • Recognized by AHA and Governor Mary Fallin for efforts to raise awareness of SCA

  • Presented two $1,000 college scholarships

 

2014-2015 Projections

  • Chase Morris featured by American Heart Association, August 2014 Stories from the Heart

  • Hosted first youth heart screening in November, 2014

  • Sponsor five additional heart screenings prior to June 2015

  • Hosted first annual benefit golf tournament

  • Establish public relations campaign for community awareness of SCA

Healthy Heart Platinum Sponsor - $10,000

  • Placement of corporate logo and/or company name on all literature, fliers and promotional materials related to Youth Heart Screenings

  • Company banner prominently displayed at all Youth Heart Screenings and CMF events

  • Logo on all CMF Youth Heart Screening marketing materials

  • Mention of partnership in all media related to CMF

  • Logo on CMF t-shirts

  • Logo and link on Play for Chase website with up to a 100-word description

  • Opportunity to distribute information and/or giveaways at all Youth Heart Screenings

  • Appropriate recognition in social media posts (Facebook and Twitter) during pre-event postings

  • 2015 golf tournament – 2 foursomes and hole sponsor signage (with opportunity to ‘sponsor hole’ with staffed table, giveaways/contests and handouts).

  • Seat on the Board of Directors
